REQUIRED FORMS

1) Read our Rules & Policies for Overnight Trips 2022.  (You do not need to sign or submit this form - read carefully and keep for your records)

2) Complete the ONLINE form: Student Medical Information

​

3) If your child requires medications on the trip, print and submit hard copy of: Parent Permission for Student to Carry & Self-Administer Medication 

​

4) Final Step: Sign and submit a hard copy of Parent & Student Signature Contract  - GVHS Music Trip 2022
